Health Forms

All health forms and requirements, including immunizations, must be completed before you start school in the fall.

An incomplete medical record may delay your registration and/or housing request.

The following forms must be completed and returned to JWU's Health & Counseling Services prior to the start of school:

Form 2, the Physical Examination form
Form 2A, the Physical Exam form for athletes (required if you plan to compete in JWU athletics)

Forms must include:

• Proof of your immunizations
• Physician’s signature
• Date of the physical exam before you return it to the university

If you are under 18, your parent’s or legal guardian’s signature is required.

You will NOT be able to register for the next term’s classes if you do not complete all outstanding requirements prior to the end of your first term of enrollment.

Alcohol.edu As part of our comprehensive alcohol prevention program, students must complete Alcohol.edu, an online, non-opinionated, science-based course taken by thousands of students each year.

You will receive an email with instructions detailing how to complete this course.
