what immunizations are required before
I register for classes? You will need to provide documentation
showing that you have had the following immunizations:
- 3 tetanus-diphtheria vaccines (DTP/DTaP/Td) with one Td booster
dose within last 10 years (required).
- Completion of Measles, Mumps, Rubella (MMR) series. This is
2 doses, both of which must be administered after the 1st birthday.
- Completion of Hepatitis B (Hep B) Series. This is a 3 dose series.
- Completion of the Varicella (Chicken Pox) vaccine series. (Two
doses are required if the first dose was given after the 13th
birthday). Documentation provided by your doctor of disease is
also acceptable.
- Negative TB test or chest X-Ray within the last 6 months.
does a doctor have to fill out my physical form? A Physician,
Nurse Practitioner or Physician’s Assistant needs to fill
out and sign the physical form before Check-In.

i'm concerned about confidentiality. who will have access to
my medical information? All medical records will be kept confidential.
The professional staff of Health & Counseling Services adheres
to the Guidelines & Standards for College Health Programs as
defined by the American College Health Association (1999):
Patient information is not revealed to any source outside the health
service, within or outside of the college, without the patient’s
informed consent. The only exception would be in cases of extreme
urgency where there is an obvious “need to know.” Policies
also conform to applicable legal statutes concerning the release
of confidential information when required by state or federal law.

will I be able to see a doctor on campus? Appointments are
available daily with either the physician’s assistant or doctor.
Consultation with the Medical Director is available as needed. Uncomplicated
visits requiring no labwork are provided free of charge. Should
you need medical attention beyond the scope of the basic services
provided on campus, the Health Services staff can make arrangements
for you to be treated at a medical facility in the community. Students
are responsible for payment of medical fees for services provided
off-campus.
is there a fee for services? There are fees for some vaccine-related services offered. All payments must be in cash.

how do I find out about student health insurance?
You will receive a brochure and identification card from Koster
Insurance once classes have started. In the meantime, if you have
