Only students who have opted to participate in Standard Check-Out are permitted to submit an appeal. If you completed an Express Check-Out, you cannot appeal any charges to your account* - refer to your closing documents. 

*-Graduating students who properly signed up for Express Check-Out and signed the graduating student log are permitted to submit an appeal form. Non-graduating students who signed up for Express Check-Out may not submit an appeal form.

If you participated in Standard Check-Out and are requesting an appeal to charges assessed to you, please fill out the form below and click submit. Appeals will not be accepted after July 1, 2011.
