The goal of Health Services is to maintain those conditions
which promote the total wellness of all students. It is our purpose
to encourage each student to realize optimum physical, emotional,
intellectual, social and spiritual wellness, while assisting each
student in reaching his/her academic goals. A university nurse is available to provide basic medical screening services
to enrolled students. These visits will cover basic medical diagnosis and care only.

If possible, bring your insurance information with you. The staff
at the hospital will determine whether to bill you, your parents
or your insurance carrier for the treatment. The university is not
financially responsible for your medical expenses. Transportation
can be arranged through Residential Life or Safety & Security.
physicians
When necessary, the nurse will refer students
to the university doctor for diagnosis and treatment. Students must be pre-screened by the nurse prior to consultation with the on-call physician.
The university physician provides health services to students during the academic/school year. A fee will be charged for required Admissions physicals and immunizations. The physician will directly bill students and/or their insurance for all services other than basic medical diagnosis and care pre-screened by the nurse. Bring your university and insurance information with you when you seek treatment.
medical records As a prerequisite for Check-In, the university and Florida Department of Health requires that all new, full-time students (residents and commuters) submit documented proof of the following:
- A physical exam performed within one year of entrance date including the physician's signature, address and date of the exam
- Two MMR (Measles, Mumps and Rubella) shots: two shots required
after first birthday; if born before 1956, only one is required
or Positive Titers (include copies of reports)
- Three doses of the Hepatitis B vaccine or Positive Immunity
(include copy of report) or signed
waiver (43K PDF), accepted at North Miami Campus only
- Meningitis vaccine or signed
waiver (43K PDF), accepted at North Miami Campus only
- Emergency contact person with phone number
- Negative PPD/CXR (within the last six months, tuburculosis test/chest
X-ray, for international students only)
All medical information is confidential and will not be released without your written consent, unless otherwise required by law.
health insurance All matriculating day school students and
all international students will be enrolled in the university's Student Health Insurance Plan for the period September through May. Coverage will cease as defined in the insurance brochure for terms students do not begin. Insurance brochures will be available at the Health Services station during Registration and at Student Financial Services during the academic year.
Day students who are not taking classes during the summer will be given the opportunity to continue coverage from June through August by the insurance provider. For information about the fees associated with this extension, please contact the insurance provider directly.
severe allergic/chronic medical conditions You should also submit your physician's documentation of any allergic or chronic medical conditions to Student Success. If you have been medically identified as being prone to anaphylaxis (severe allergic reaction), you may authorize the university to administer epinephrine in an emergency. Although this information may have been shared with other university personnel, students must notify Student Success of severe allergies or chronic medical conditions immediately following Registration.
